New Patient Intake Form: Psychological & Educational Testing
Assessments are typically booked within 4-8 weeks, depending on type.
Please complete our intake form in its entirety prior to your child’s evaluation to help us understand your child’s needs and your concerns. This information allows us to determine whether an assessment is appropriate and how we can best support your family. The form takes approximately 10–15 minutes to complete, and all information provided is kept strictly confidential and protected under the Personal Information Protection and Electronic Documents Act (PIPEDA) regulations.
